Cut med spa no-shows with four things: a clear cancellation policy, a deposit or card on file for new clients, text reminders before the visit, and a waitlist that refills open slots. Zenoti's 2025 data put med spa cancellations at 14% and no-shows at 4%, with the biggest drop-off before a patient's second visit.

How big is the no-show problem at a med spa?

Smaller than it feels on a bad day, but it adds up. Zenoti's medspa benchmarks, built from anonymized data across thousands of North American locations, put the 2025 cancellation rate at 14%, down from 16% in 2024, and no-shows at 4%. Zenoti sells spa software, so read it as one vendor's data, not a census.

Example: a spa with 300 appointments a month at those rates sees about 42 cancellations and 12 no-shows. Multiply the slots you cannot refill by your average ticket to see what it costs you.

Does a deposit or card on file stop no-shows?

It helps most with new clients and long or high-value treatments, because people treat a booking differently when money is attached. The policy matters as much as the deposit: short, clear, shown at booking, repeated in reminders and applied the same way every time.

  • A deposit or card on file for first visits and long treatments.
  • A cancellation cutoff in plain words, like 24 or 48 hours.
  • Exceptions for real emergencies, decided by a person.
  • The policy at booking, in the confirmation and in every reminder.
  • A lawyer's read before you charge any fee.

Most spa booking platforms handle deposits and reminders. Compare them in our med spa booking software guide.

When should reminders go out?

Send a confirmation at booking, a reminder 48 hours before with an easy way to reschedule, and a short one 24 hours before. Moving a visit is far better than missing it. Here is an example schedule.

When What to send Why
At booking A confirmation, the policy and prep instructions Sets expectations
48 hours before A reminder with confirm and reschedule links Moving beats missing
24 hours before A short reminder Catches the forgetful
2 hours before, first visits only "See you at 3. Parking is in back." Calms first-visit nerves
After the visit Aftercare and a rebook link Starts the next visit
After a no-show One kind message with new times Saves the client

Text reminders do move the number. A Cochrane review of 8 trials on healthcare appointments found attendance of 78.6% with text reminders and 67.8% with none.

How do you protect the second visit?

Zenoti's data has the most useful number in this guide: 37% of patients who book a second appointment cancel before it happens. Once they complete that second visit, cancellations fall to 4%. So the second visit is where your effort pays off most.

  • Book the second visit at checkout, before the client leaves.
  • Send a short check-in between visits, in your provider's words.
  • Remind them why the series matters, without medical claims.
  • Make rescheduling one tap, so they move the visit instead of dropping it.

How do you refill a cancelled slot?

Keep a waitlist by service and provider. When a slot opens, text the waitlist, and the first person to reply gets it. That turns a late cancellation into a booked chair instead of an empty one. It works best when it runs on its own, because cancellations land when your front desk is busiest.

This is the kind of job our automations handle: the cancellation comes in, the waitlist gets a text, and the booking lands in your platform.

Custom vs rented vs doing it yourself

Booking platform features Built around your platform Doing it yourself
Reminders Built into most plans Uses your platform's reminders, adds what is missing Manual texts
Deposits and card on file Built into many plans Stay in your platform Payment links by hand
Waitlist refill Varies by platform Texts the waitlist the moment a slot opens Calling down a list
Second-visit check-ins Campaign tools, often extra Your words, sent on approval or automatically When someone remembers
Cost A monthly plan A build priced on a free call, then a flat monthly plan Front desk hours

If your platform already does all of this, turn it on and write good messages. You may not need us.

What should you say after a no-show?

Say one kind thing and offer a way back. Most people who miss a visit feel a little embarrassed, and a scolding text makes it easy for them to never return. If your policy includes a fee, state it plainly once, in the same words your policy uses. Here is an example for a made-up spa:

Hi Jess, we missed you at 3 today. We hope everything is okay. Here are two openings this week: Thursday 11 AM or Saturday 2 PM. Want one? Reply STOP to stop texts.

If the same client misses twice, a person should call, not the system. Some clients need a different time of day, a reminder by call instead of text, or a deposit going forward.

How do you know it is working?

Measure before you change anything, then compare month to month. Four numbers are enough, and most booking platforms report them.

  1. No-show rate: no-shows divided by booked appointments.
  2. Late cancellation rate, using your own policy cutoff.
  3. Refill rate: cancelled slots that the waitlist filled.
  4. Second-visit rate: first-time clients who completed a second visit.

If no-shows drop but late cancellations climb, your reminders are working and your waitlist is the next fix. If second visits stay low, look at what happens at checkout.

What should you avoid?

  • Treatment details in texts. Keep reminders to the time, the place and prep.
  • Fees that are not in your written policy.
  • Rescheduling that only works by phone during business hours.
  • Promotions to people who did not agree to them. STOP has to work every time.

Questions owners ask

What is a normal no-show rate for a med spa?

Zenoti's 2025 benchmarks, drawn from thousands of North American medspa locations, put no-shows at 4% and cancellations at 14%. Your own rate may differ, so measure it for a month before you change anything.

Should a med spa charge a deposit?

Many do for new clients and longer treatments. Put the amount and the refund rules in writing at booking and in every reminder, apply them the same way every time, and have a lawyer review the policy.

When should med spa appointment reminders go out?

A common pattern is a confirmation at booking, a reminder 48 hours before with a reschedule link, and a short one 24 hours before. First-time clients can get one more on the day.

Why do med spa clients cancel the second appointment?

Zenoti found that 37% of patients who book a second appointment cancel before it happens. Booking it at checkout, a check-in between visits and easy rescheduling all help keep it.

Can Elevate automate reminders and a waitlist for my spa?

Often, yes, on top of the booking platform you already use. Every build is priced on a free 15 to 30 minute call, with one flat price in writing before work starts.
